Consultation Services
Information on Consultation Services
Once your blood test results are available Dr Maysam will review the results during every consultation, Dr. Maysam will review your health history and conduct a comprehensive hormone blood lab panel. This information will be used to recommend the best BHRT/ Bioidentical Hormone Replacement Therapy and Testosterone Replacement Therapy treatment to provide “optimal hormone levels”.
Follow-up appointments will be made every three months for the first year to make sure everything is on the right track.
Booking an Appointment Consultation
In order to confirm your appointment, consultation you will need to complete these steps:
- Select your consultation date, provide your credit card information to be held on file.
The first visit fees is $375, then $250 at each follow up visit @3, 6 and 9 months.

Follow up fees - $250 each at 3, 6, 9 months
- Complete the Intake Forms.
Once these two steps have been completed, our office will review your file and give final approval for your appointment date and time.
If patients thinks they need to be approved they will not fill out the forms.
Please complete the intake form + consent + privacy form, we will then give you a blood requisition.
Insurance Coverage
These consultations are not covered under OHIP. It is up to you to contact your insurance provider to determine your coverage.
Consults & prescriptions may be covered by your private insurance, all depends on the plan you have, please submit your receipts directly to your own insurance company (this is not a public health service & not covered by OHIP). Dr. Maysam does not fill out insurance forms.
Labs
Lab tests will be required before each consult and before the prescriptions can be issued for Dr Maysam to access the treatment proscribed. Most of the lab tests that are ordered will be covered under OHIP where applicable.
